Residential Habilitation & Support (RH)

Residential Habilitation and Support Services provide from a limited few hours up to a full day (24-hour basis) of services and/or supports which are designed to ensure the health, safety and welfare of the participant, and assist in the acquisition, improvement, and retention of skills necessary to support participants to live successfully in their own homes.

Community-Habilitation Individual or Group (CHIO or CHGO)

Community Habilitation (provided on an individual or small group basis) are services provided outside of the participant’s home that support learning and assistance in the areas of self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills. Community-based activities are intended to build relationships and natural supports.

 

Facility-based Habilitation Individual or Group (FHIO or FHGO)

Facility Based Habilitiation (provided on an individual or small group basis) are services provided outside of the participant’s home in an approved facility that support learning and assistance in the areas of: self-care, sensory/motor development, socialization, daily living skills, communication, community living, and social skills.

 

Respite (RSP)

Respite is a service provided to participants on a short-term basis in order to provide temporary relief to those unpaid persons normally providing care. Respite  can be provided in the participant’s home or place of residence, in the respite caregiver’s home, in a camp setting, in a DDRS approved day habilitation facility, or in a non-private residential setting (such as a respite home).

 

Attendant Care (ATTC)

Attendant Care services primarily involve hands-on assistance for aging adults and persons with disabilities. These services are provided in order to allow older adults or persons with disabilities to remain in their own homes and to carry out functions of daily living, self-care, and mobility.  Attendant Care services are limited to 40 hours a week.

 

Homemaker Services

Homemaker services offer direct and practical assistance consisting of household tasks and related activities. The services assist the individual to remain in a clean, safe, healthy home environment and are provided when the individual is unable to meet these needs or when an informal caregiver is unable to meet these needs for the individual.  Homemaker services are limited to 10 hours a week.

 

Residential Based Habilitation (RBHA)

 Residential Based Habilitation provides assistance with the acquisition, retention or improvement in skills related to activities of daily living and the social and adaptive skills necessary to enable the individual to live in a non-institutional setting.
